THE CLAMP PLATES BETWEEN THE DRIVE SHAFT AND CARRIER YOKE COULD CRACK. A BROKEN CLAMP PLATE WOULD RESULT IN THE YOKE FLYING OFF AT THE DETACHED POINT. THIS COULD CAUSE INJURY FROM FLYING FRAGMENTS TO PERSONS IN THE VICINITY. ALSO, THE DETACHED INNER SHAFT COULD INTERFERE WITH THE BRAKE SYSTEM AIR SUPPLY HOSES AND EVEN THE TRACTOR TIRES. THIS COULD CAUSE LOSS OF VEHICLE CONTROL AND A POSSIBLE CRASH. CORRECTION: NEW CLAMPING PLATES AND CAP SCREWS WILL BE INSTALLED AS REQUIRED. NOTE: NO VEHICLES IN CANADA.

ON TRUCKS WITH MACK FA(W)616 OR 617 SERIES FRONT AXLE, THE CROSS STEERING LEVERS MAY BE IMPROPERLY HEAT-TREATED. THIS COULD REDUCE THEIR FATIQUE LIFE AND RESULT IN FAILURE. SUCH A FAILURE COULD LEAD TO LOSS OF VEHICLE CONTROL WITHOUT PRIOR WARNING. CORRECTION:NEW LEVERS WILL BE INSTALLED.

THE BROKEN CROSSMEMBER CAN PROTRUDE ABOVE THE TOP OF THE FRAME AND INTERFERE WITH THE TRAILER CHASSIS COMPONENTS. THIS COULD ADVERSELY AFFECT HANDLING WHEN TURNING. ALSO, A COMPLETELY SEPARATED CROSSMEMBER WITH ATTACHED HEIGHT CONTROL VALVES COULD AFFECT THE HEIGHT OF THE SUSPENSION AIR SPRINGS. THE RESULTING LEAN COULD CAUSE LOSS OF CONTROL.
