On certain vehicles, the high pressure fuel line from the pump could fail. Failure of the line introduces atomized fuel into the engine compartment which could contact hot surfaces and ignite. A support bracket was developed by Cummins to alleviate the vibration on the line. Correction: Owner notification including repair instructions will be supplied. Fleet customers are to install support bracket to alleviate vibrations on the fuel line.

On certain low floor transit buses, the flip-out ramp designed to assist riders with wheelchairs or mobility aids enter and exit the vehicle may fail. The ramp hinges may have inadequate spot-welds unable to withstand the designed load limit of the ramp assembly. Failure of the hinge assembly spot-welds could allow the ramp to separate from the vehicle, resulting in injury to the user. Correction: Owner notification including repair instructions will be supplied. Fleet customers are to install hardware to the hinges which will make the spot-weld redundant.

Loss of rear brake application pressure could increase stopping distances, causing a crash. On certain transit buses, a weld was omitted from the front axle radius rod brackets during assembly. This missing weld decreases expected service life of the brackets and may eventually lead to separation of the radius rod bracket and the frame. Failure of the bracket could result in a loss of vehicle control. Correction: Owner notification and repair instructions for front axle radius rod bracket replacement will be supplied.

On certain buses equipped with a Cummins ISC/ISL engine, improper installation of a boss plug during engine assembly could create stresses in the cylinder block casting. These stresses could eventually create open cracks in the cylinder block or result in less than adequate clamping pressure around the plug. When the plug loses it's clamping pressure, it will loosen from the cavity, causing an oil leak. This leakage of oil near the exhaust manifold and turbocharger has the potential to ignite and cause a fire in the engine compartment. Correction: Cummins will contact owner and to install a steel cup plug boss cap which is attached over the boss/cup plug location with an industrial sealant.
